Form and Contents of the Decision
1. The decision contains:
a) the composition of the committee;
b) the names of the parties;
c) a summary of the facts;
d) the legal reasons for the decision;
e) the provisions on which the decision was based;
f) the terms of the decision;
g) notice of the channels for appeal.
2. The decisions are signed by the committee secretary. Subsection 3. Proceedings before the chairman of the Disciplinary Committee acting alone
